The Centers for Medicare and Medicaid Services (CMS) proposed changes in the past, with the transition to a prospective payment system (PPS) approximately 15 years ago, however, these were never adopted, holding providers to conditions of participation that were written in 1989. Many industry experts are supportive of an overhaul of the CoPs to better reflect the modernization of these regulations and reflect current industry practice. The changes to CoPs emphasize care coordination and communication strategies that focus on both timely and appropriate delivery of interdisciplinary care with a focus on optimal outcomes.
This webinar will provide participants with a detailed review of the new CoPs. Specific examples of how these changes will impact current industry activities will be provided, including expansion of the patient rights requirements, care coordination and integrated communication standards for the home health agency. Detailed discussion of the new data driven quality assessment and performance improvement (QAPI) program requirement will be provided. A Care Coordination Tool will be provided to assist agencies in ensuring that systems and clinicians are able to meet standards established by these updates to conditions of participation in the Medicare Home Health program.

Speaker: Cindy Krafft PT, MS, HCS-O brings more than 20 years of home health expertise that ranges from direct patient care to operational / management issues as well as a passion for understanding regulations. Years spent in the direct patient environment solidified that this was the optimal setting for the provision of effective and efficient interdisciplinary care as it focuses on functionality and patient centered care delivery. Recognition that the providing care in the home environment presents both unique opportunites and challenges is clearly evident in both training and consultation activities.
For the past 15 years, Cindy has been a nationally recognized educator in the areas of documentation, regulation, therapy utilization and OASIS. She currently serves on multiple Technical Expert Panels with CMS Contractors working on clinical and payment reforms such as bundled payment care initiatives and alternatives to the current relationship between therapy visits and episodic payment. She was instrumental in reducing the burden placed on the industry due to the timing of functional reassesments by diliently working with both APTA and CMS. Her focus is on providing the knowledge and tools critial to the post acute continuum of care to operationalize external requirements while keeping the driver of care where it needs to be - the needs of each patient being cared for in the home setting.
Cindy has been involved at the senior leadership level for the Home Health Section of the American Physical Therapy Association for more than 10 years and is the immediate Past President and current Federal Affairs Liaison of that organization. She is on the editorial board of Home Healthcare Now and a frequently sought out expert contributor for other home health publications. She has written 3 books - The How-to Guide to Therapy Documentation, An Interdisciplinary Approach to Home Care and the soon to be released Handbook to Home Health Therapy Documentation - and co-authored her fourth, The Post-Acute Care Guide to Maintenance Therapy with her business partner Diana Kornetti PT, MA, HCS-D.
